Dentist and Registered Dental Hygienist licensure renewals are due Sept. 30. To be sure your credential does not expire, begin the renewal process at least ten business days before the due date. If you receive your initial credential within the months leading up to the due date, you are still required to renew.
Requirements for renewal include $123 (an additional $25 if late), signature certifying completion of continuing education and current CPR/AED certification.
You can renew online or request a paper renewal form. Allow up to 2 business days for processing of online applications.
