The Centers for Medicare and Medicaid Services to enroll or opt-out of Medicare has been delayed again to Jan. 1, 2019.
All providers make their decision and submit their applications to allow time for processing.
In order for Medicare to pay for prescriptions you write to your Medicare eligible patients, you must either enroll as a Medicare provider, enroll solely to prescribe drugs to those Medicare patients or opt out of Medicare completely.
In order for Medicare to pay for prescriptions under Medicare Part D, a dentist must do one of the following:
- Fully enroll as a Medicare provider by completing form 855l
- Enroll solely to prescribe drugs for patients on Medicare by completing form 855O
- Opt-out of the Medicare program by completing the affidavit
To enroll, visit the federal government’s Medicare Provider Enrollment, Chain and Ownership System at pecos.cms.hhs.gov.
To opt-out, an affidavit form and National Provider Identifier need to sent to the following Part B entity:
